Game Sheet
Print large, clearly, and hard with a good pen. (Remember these are being faxed)
1. Rosters
List goalies first, “S” for starting goal tender and “Alt” for back up goal tender if more than one dressed. List player’s names by Jersey number, then alphabetical order, last name first.
After name write “C” for Captain, “A” for alternate, “AP” for Affiliated player, “OA” for overage player.
For players not on the bench a small single line through name and “abs” for absent, “inj” for injured or “susp” for suspended in place of and/or beside jersey number.
2. Team Officials
Print names; DO NOT have them sign here. Managers name on the game sheet ONLY if they are carded.
Verified by: (Signature) REMIND coaches that they are signing that the roster IS complete and correct. GAME NUMBER TO BE WRITTEN ON SHEET. Lots of room at top of the page.
3. Scoring
All fields MUST be filled in: “Period” scored in, “Time” goal scored, who “Scored” and who “Assisted”.
When ref gives you a number, take a second and check to see if that number appears on roster, refs make mistakes too and a tucked in jersey can be hard to read. What if #16 (the number the ref gave you) is not on the roster but #76, #18 and #26 are. Who scored?
4. Penalties
All fields MUST be filled in “Period” received in, “Number” who received, player penalty “Served by”, what the “Offence” was, “Minutes” to be served, “Off” the ice time, penalty “started” time, “On” the ice or Player let out at. Include ALL Game Misconduct Penalties -“GM” in offence field, showing 10 min time to player or team official. Manager MUST fax incident report to Gov. for ruling for all penalties 5 minutes and over.
5. Game Officials
“Referee name” have them fill in at beginning or end of game. MUST include AAHA number.
“Linesman name” have them fill in also, Print names for “Official Scorer”, “Penalty Timekeeper” and “Official Timekeeper” (NO signatures here, nobody can read them afterwards)
6. Time (Any mistakes here and the game could be ruled invalid and the game rescheduled)
“Time Game Started” (include warm up) Actual time not when it was scheduled to start. Rule 5a) At the start of ALL CAHL League and Playoff games the clock will be set to a five minute warm-up period. The clock shall be started immediately at the commencement of the scheduled time slot, or when the ice is available for both teams to commence skating, whichever is the latter. At the end of the five-minute warm-up period the clock shall be reset to twenty minutes and the game started.
Should the ice not be available at the scheduled start time, the visiting team is entitled to the scheduled game duration. This means that if the game is delayed 10 minutes as a result of a home team (ice or team not ready), the visiting team can expect the game duration to be the same and the end time to be 10 minutes later. However, if the ice and home team are ready and the visiting team fails to be ready for the scheduled start time, this should be noted on the game sheet and the game time and duration would remain the same.
“Time Clock Dropped” Rule 5h) Stop time is to be used in all periods of all games. When five minutes remains on the ice permit and after the first stoppage of play thereafter, the time clock will be set to two minutes and the remainder of the game will be played stop time. All Member Associations must declare at the Annual General Meeting whether they will follow this regulation or guarantee to provide enough ice time to complete the game. Know whether your association has declared to REDUCE (drop time) or PLAYOUT games. You must follow that declaration.
Example for REDUCE associations: 2 hours has been scheduled for a game (8:30 am to 10:30 am), there is 15 minutes on the clock but the time is 10:23 am at the first stoppage of play AFTER 10:25 am. Set clock to 2:00, record “Clock dropped time” 10:26 am and play stop time for the remainder.
“Time Game Ended” Actual time, not what the ice was scheduled for.
Any of the above information missing or illegible can result in fines to your association.
7. Completion (Suggested. Done by many referees at the end of the game.)
Total roster, total persons on bench confirm they match, write and circle that number.
Single line through unused scoring fields, total goals and circle that number.
Single line through unused penalty fields, total minutes and circle that number.
Game sheets are to be faxed to the Governor within 48 hours of the game being played
